Guidelines & Application Procedures
The Tucson Osteopathic Medical Foundation is offering awards to
residents of Arizona attending AOA accredited colleges of osteopathic
medicine. Applicants from southern Arizona will be given SPECIAL
consideration. All candidates will be selected on an objective
basis without discrimination on the basis of race, sex, age, creed,
national origin or physical disability.
The Founders' Awards honor the five osteopathic physicians who
were the original incorporators of the southern Arizona's first
osteopathic hospital, Tucson General Hospital. The loans are named
for these five men: John W. Duncan, D.O.; Homer M. Fredericks,
D.O.; Robert W. Grant, D.O.; Coy L. Purcell, D.O.; and Carlton
E. Towne, D.O.
Eligibility
•Applicant must be an Arizona resident, defined as any
person who maintains a twelve-month place of residence in Arizona,
prior to entering a college of osteopathic medicine.
• Residents of the seven southernmost counties of Arizona
will receive first consideration. The seven southernmost counties
of Arizona are defined for the purpose of this program as the
counties of Cochise, Graham, Greenlee, Pima, Pinal, Santa Cruz
and Yuma.
•Applicant must be enrolled in a college of osteopathic medicine
for the 2007-08 academic year. (Applicants may be in any year
of the academic predoctoral program.)
•Applicant must be seeking a D.O. degree.
•Applicant must complete and mail application by March
15, 2007.
•Applicant must submit two letters of recommendation by
March
15, 2007.
•New applicants that pass screening process must have a personal
interview with the Founders' Awards Committee.
•Applicant must provide a copy of the 2007-08 Graduate and
Professional Student Financial Aid Service application (or such
other application form as is required by the college of osteopathic
medicine at which the student is applying or enrolling) by March
15, 2007.
Terms and Conditions
1. Awards in the form of loans may vary in size. A total of $75,000
will be available for all awards made during the 2007-08 academic
year.
2. Under the terms of the loan program, students will be awarded
loans at interest rates comparable to currently available educational
loan programs.
3. Loan recipients will be forgiven 20% of the loan for every
year the loan recipient maintains his/her principal medical practice
in one of the seven counties of southern Arizona delineated above.
Notwithstanding the forgiveness of the loan provision, no portion
of a loan already paid back will be reimbursed to an applicant.
4. The loan is interest free during the formal in-school period,
internship and residency.
5. Loan payments and interest accumulation may be deferred during
other advanced professional training and military service at the
sole discretion of TOMF.
6. After deferred period has terminated, if the recipient has
not established a primary practice in southern Arizona, full payment
will be required (principal plus interest) within a maximum repayment
schedule of five years.
7. The loan must be co-signed by an Arizona resident.
8. A promissory note must be agreed to and signed by each recipient.
9. It is the responsibility of the loan recipient to annually
provide the Tucson Osteopathic Medical Foundation with a statement
of status. In the case of currently enrolled students at colleges
of osteopathic medicine, this statement should consist of a signed
letter from the registrar's office attesting that the recipient
is a student in good standing.
In the case of interns or residents, this statement should consist
of a signed letter from the office of the Director of Medical
Education or the office of the Medical Director. This statement
must be received at the Foundation by October 1 of each year.
As outlined in section 11, of these terms, failure to provide
such a statement will cause the loan to become due in the case
of a student. In the case of an intern, failure to provide such
a statement will cause the loan to become active, with interest
beginning to accumulate and payments due.
10. It is the responsibility of the loan recipient to maintain
a current, accurate mailing address with the office of the Tucson
Osteopathic Medical Foundation. This is the address to which all
official notices will be sent. The Foundation cannot take responsibility
for notices which are not delivered due to recipient failing to
maintain a current address.
11. The whole loan becomes due, with or without notice from the
lender, if the recipient:
A. Withdraws, is dropped or suspended from the college of osteopathic
medicine.
B. Fails to make payment within 120 days.
C. Files a bankruptcy proceeding, or assigns assets for the benefits
of creditors.
D. Makes a false written statement in applying for the loan, or
requesting an extension or suspension of the loan payment.
COLLECTION PROCESS
Collections will be handled by the Tucson Osteopathic Medical
Foundation. However, TOMF may, at its sole discretion, assign
the loan to a third party.
A payment book will be generated for monthly payment, or TOMF
may bill the recipient monthly at its sole discretion.
Physicians taking advantage of the forgiveness provision must
demonstrate to TOMF's satisfaction that more than 50% of their
practice is situated in southern Arizona.
MAILING APPLICATIONS
Applications mailed to any other address may not be accepted.
The Tucson Osteopathic Medical Foundation reserves the right not
to process applications found to be incomplete as of the application
deadline. The selection committee reserves the right to request
such additional information as it deems necessary.
A decision on your application will be made by MAY 30, 2007.
If you are selected, funds will be available after July 1, 2007
and will be paid directly to the institution in which you are
enrolled.
