The Tucson Osteopathic Medical Foundation (TOMF) presents the return of Social Determinants of Health (SDOH) innovation as the focus of its 2026–2027 grant cycle, awarding up to six, $5,000 grants to eligible 501(c)(3) organizations in Pima County whose programs advance meaningful progress within the SDOH innovation framework. 2025-2026 Grant Cycle The Tucson Osteopathic Medical Foundation (TOMF) selected Social Determinants of Health (SDOH) innovation as the focus of its 2025–2026 grant cycle, awarding $25,500 to eligible 501(c)(3) organizations in Pima County whose programs advance meaningful progress within the SDOH innovation framework. |

$7,500 for fresh produce purchases and literacy materials to expand the Your Healthy Home Market program. | 
$5,500 to support the purchase of Victor Reader Stream 3 devices for use in the lending program.

$7,500 to support the purchase of materials for the home repair program. |
