The Tucson Osteopathic Medical Foundation (TOMF) selected Social Determinants of Health (SDOH) innovation as the focus of its 2025–2026 grant cycle, awarding $25,500 to eligible 501(c)(3) organizations in Pima County whose programs advance meaningful progress within the SDOH innovation framework. |

$7,500 for fresh produce purchases and literacy materials to expand the Your Healthy Home Market program. |
$7,500 to support the purchase of materials for the home repair program. |

2022-2025 Over $80,000 was awarded to 11 different organizations with innovative programs in the target areas of mental/behavioral health and substance use disorders.
